The shortage of cadaveric organs for transplant has persisted constantly since early 1980s. This scenario has necessitated the search for alternative organ procurement policies. Any decision made in this regard is very crucial as it will affect the well-being of many patients globally. This paper focuses on three proposals fronted by the article. The article criticized the current altruistic system of organ procurement and blamed it for the current short supply of transplant organs. Altruism is a concept that requires people to donate their organs freely and based on their own volition.

The article roots for a market-based organ procurement system that operates on the basis of willing buyer willing seller. The article argues that this system has the potential to generate the required number of transplant organs at the required rate. However, the system raises many ethical and economic concerns. A market-based organ procurement system favors the rich and denies the poor the opportunity to receive organs. Furthermore, the system erodes the principle of medical ethics and goes against the requirements of competency and voluntariness in organ donation.

The article also roots for reassignment of property rights procurement system. This system paves way for a negotiated agreement between the donor and the receiver. Unlike the former, this system does not disregard ethical and economic principles based in most western traditions. The system also borrows heavily from the current altruistic system. If well implemented, the system has the potential to increase the rate of organ donation and address challenges bedeviling the current system.
